Job Summary and Qualifications
The Total Joint Program Nurse Navigator (TJNN) is responsible for overall coordinating and implementing the centers Total Joint Replacement Program. The TJCN will be guiding the total joint patient and their family through the program, from preoperative assessment and education through the continuum of care. This individual will have the ability to interact with patients and their families, physicians, and other care partners, such as physical therapy. The TJCN will act as a liaison between all departments of the surgery center and advocate for the patient as needed. Regularly collects data through chart review and computer generated reports. Presents data and reports to Quality Committee and other leadership groups as directed. Develops, facilitates, and monitors selected performance metrics and ensures compliance with all activities required to achieve and obtain/maintain Orthopedic Certification with AAAHC. Works closely with orthopedic surgeons to assist with data collection and community outreach.
Education & Experience
- 5 years applicable clinical experience.
- Minimum 2 years in an acute care orthopedic setting, preferred.
- Supervisory experience preferred.
- Operational and strategic development experience preferred.
- Associate Degree in Nursing or Nursing Diploma required.
- BSN required. (internal candidates only - experience may substitute for degree)
